L-glutamine is an amino acid found abundantly in our blood and body fluids. It is the building block for protein and has an important role in our immune system. Our body manufacture L-glutamine naturally. We also get it from food and supplement.  

I do not know of any literature that links L-glutamine to penile shrinkage.

It may not be easy to differentiate blood in the urine from concentrated urine. A quick urinalysis in the doctor's clinic will tell you if it is indeed blood. The ability to fully retract the foreskin increases with age. I usually tell my patients that full retraction is possible for 10% of boys at 1 year, 50% of boys at 10 years and 99% of boys at 17 years. 
Treatment may not be necessary unless he has true phimosis, difficulty passing urine, or developed balanitis because of the phimosis. A Urologist will be able to advise if treatment is necessary after review.
